​Implementing the direction of the Provincial People's Committee in Official Dispatch 3796/UBND-VX dated August 07, 2021 on Covid-19 vaccination for investors, experts and foreign employees in the locality.

The Foreign Affairs Department respectfully requests the Industrial Zones Authority, the Department of Labor, War Invalids and Social Affairs and the People's Committees of districts, towns and cities to send notices to enterprises, units, organizations and foreigners living, working and studying in the province to compile the list of Covid-19 vaccination for the phase 5 (list of statistics by country and territory), specifically as follows:

1. Subject:

Foreigners (from full 18 years of age or older) living, working and studying in Binh Duong province HAVE NOT BEEN VACCINATED AGAINST COVID-19.

2. Registration time: From September 15, 2021 to 12:00 on September 20, 2021.

3. Instructions to register for vaccination: Companies/Individuals register the following steps:

Step 1: Declare the list according to the attached form in Appendix I (Enterprises must take legal responsibility for the authenticity and legality of the list of foreigners when registering to send documents to the Foreign Affairs Department. If you are an individual, register with your personal email and take legal responsibility for the accuracy of your registration information). The Appendix I form is posted on the website of the Foreign Affairs Department at songoaivu.binhduong.gov.vn.

- Send by email according to the following instructions:

+ If it is an enterprise, send a photocopy of the enterprise's registration letter and the registration list with the enterprise's red stamp, and at the same time send the soft file of the registration list (file in *.xls, *.xlsx format) with email name and default file name is Enterprise Name – Country/Territory

(For example: ABC Company Limited is a Vietnamese enterprise, the email name is ABC–Vietnam  .

+ If you are an individual, send the soft file of the registration list with email name and default file name is Citizen – Nationality

(For example, if you are a Japanese citizen, the email name is   Japanese citizen

- Send to the corresponding email address as follows:

+ For foreigners from other countries and territories: Email to sngvbd.vaccine@gmail.com.

+ For Chinese citizens: Email to sngvbd.tdm@gmail.com

+ Other notes: If the enterprise has Chinese citizens and citizens of other countries and territories, then register Chinese citizens and citizens of other countries and territories separately according to the above 02 emails.

Step 2: Request the Enterprises/Foreigners to fully declare the mandatory information according to the QR Code to update it into the hospital system software when vaccinating (QR Code according to the location in Appendix II). In which, note:

+ The information section "company name" declares the correct name registered in the business registration certificate/investment certificate so that the information is consistent.

+ The Foreign Affairs Department is not responsible for problems arising from the discrepancy between the email of the official registration list and the declaration of the QR Code; The incomplete declaration of information will slow down the processing time of your application.

Due to the urgent nature of the above work, the Foreign Affairs Department respectfully requests the Industrial Zones Authority, the Department of Labor, War Invalids and Social Affairs and the People's Committees of districts, towns and cities to coordinate in implementation.
