Making electronic health declaration by QR Code when entering and leaving the one-stop-shop area of the Provincial Administration Center

​PORTAL - The Provincial Public Administration Center notifies the Head of the Dossier-Receiving and Result-Delivering Section (One-stop-shop section), civil servants, public employees, employees performing their duties at the one-stop-shop area (1st Floor, Provincial Administrative Center) and individuals and organizations coming to conduct administrative transactions, make business contacts, work at the Provincial Administrative Center to cooperate in the Covid-19 pandemic prevention and control.

Specifically, strictly implementing the 5K message of the Ministry of Health including: "facemask disinfection - distance - no gathering - health declaration"; performing mandatory mask wearing at the one-stop-shop area.

Instructing individuals and organizations to scan QR Code when entering and leaving the one-stop-shop area (also known as Medical Check-in, Check-out) according to the guidance of the National Steering Committee for Covid-19 Prevention and Control and instructions in Official dispatch no. 17/HCC-CNTT- KTGS of the Provincial Public Administration Center at 02 points for electronic health declaration by QR Code (Security scanner area - 1st floor, towers A and B, Provincial Administration Center).

The Head of the One-stop-shop Section directs one-stop-shop personnel to strictly carry out the reception of individuals and organizations for administrative transactions at the one-stop-shop area to ensure measures to prevent and control the Covid-19 pandemic; make health declarations and propagate, guide individuals and organizations to perform medical Check-in, Check-out, and health declaration.

The Security Team (Building Management Board) guides and supports individuals and organizations to perform medical Check-in, Check-out, health declaration by QR Code at the one-stop-shop area. Agencies and units when working with units outside the Administrative Center Building in the one-stop-shop area are responsible for instructing individuals and organizations outside to perform medical Check-in, Check-out, and health declaration as prescribed.

The implementation period is from May 6, 2021 until further notice.

Steps of QR Code scanning to perform Check-in/Check-out

Step 1: Open and install the app of "Bluezone", "NCOVI", "Vietnam Health Declaration", Zalo, etc. to scan QR Code from CH Play/Apple Store app.

Step 2: Select “Scan QR”.

Step 3: Scan the QR Code and perform medical “Check-in/Check-out”.

Step 4: Make electronic health declaration (when required)​
